- Identification of the removed material and its former location
- A statement under penalty of perjury that the material was removed by mistake
- Your name, address, phone number, and consent to jurisdiction
Repeat Infringers
We may terminate access for users who are found to be repeat infringers. We track DMCA notices and take appropriate action against repeat violations.
